Booking Instructions

Select the amount you would like to donate per dedication by selecting the appropriate ticket price below.

Each book dedication will cost the price of the ticket selected, e.g 3 dedications on the £5.00 dedication ticket will cost £15.00.

If you would like to receive a Light up a Life card in memory of your loved one at no extra cost please tick the box for each attendee (cards will be posted out to the booking addressee). Cards are not available on bauble dedications

Remember to fill in all appropriate fields for each dedication (fields with a red star are mandatory). Please check spelling is correct as this is what appears in the Book of Dedication and on the Light up a Life cards.

Due to current Covid restrictions the Book of Dedication will not be available to view at the Hospice for the foreseeable future but can be viewed online and will be updated weekly. A link to view the book will be sent on receipt of payment.

There are a maximum of 2 names per bauble dedication

Quantity 476 available	Memory Bauble	£5.00	Purchase a dedication to be placed inside one of our Memory Baubles which will decorate on one of our 2 Remembrance Trees this year. The cost is £5, and we can put a maximum of two names in each bauble.
