As a integral member of the Day Hospice and Hospice at Home Team, you will be responsible for providing support to the Clinical Services Team Lead (CSTL) and Clinical Services Staff Nurses (RN) in relation to day to day Hospice clinical activities, providing patient care within agreed policies and procedures and agreed levels of service delivery, contributing to the daily provision of care services; including supporting staff and volunteers.

The Senior Care Support Worker (SCSW) will provide support to the CSTL and RN’s in maintaining high quality palliative and end of life care to patients in the hospice and their own homes, and support to their families. Under the supervision of the RN, the SCSW will assess, plan, implement, evaluate and co-ordinate care to meet patient’s care needs encouraging re-ablement and well-being models of care. At all times the post holder will ensure patients and their carers receive the right hospice care, at the right time and in the right place, optimising patient and carer satisfaction by always facilitating individual’s preferences and a personalised care approach.

The SCSW will support the CSTL and RN’s by providing specialised palliative and end of life care which is safe, caring, responsive, effective and well-led supporting the Hospice’s compliance with all aspects of Care Quality Commission (CQC) national standards and related legislation.
