Job Summary:

The Team Lead for Community Services (planned care at home and onsite care) has a key role supporting the Head of Clinical Operations by ensuring the community clinical services are providing specialised palliative and end of life care which is safe, caring, responsive, effective and well-led supporting Mary Ann’s compliance with all aspects of Care Quality Commission (CQC) national standards and related legislation.

The post holder will ensure day to day efficient running of the services, overseeing the clinical caseloads and supporting Staff Nurses and Nurse Associates who have responsibility daily for operational service delivery which includes supporting and supervising staff and volunteers. The post holder will also be an integral member of the Rapid Response Day Team providing regular cover for this service as needed.

The post holder will work with the Head of Clinical Operations to ensure expert and specialised practice is provided and further developed to enhance clinical effectiveness, encourage re-ablement and well-being models of care. At all times they will ensure patients and their carers receive the right hospice care, at the right time and in the right place, optimising patient and carer satisfaction by always facilitating individual’s preferences and a personalised care approach.

As the Community Services Team Lead (CSTL) is a senior position the post holder will demonstrate effective leadership skills, resource management and be a credible and professional clinical role model.
