Job summary

Due to staff progression, we have an exciting opportunity to recruit a motivated and enthusiastic Band 5 to our community Services team at Mary Ann Evans Hospice, working across Hospice at Home Services and the Well Being Centre.

Mary Ann Evans Hospice is the local community hospice in north Warwickshire whose aim is to provide specialised care, support and advice to patients and their families when living with a life limiting illness.

Hospice at home provide end of life care and support to people wishing to remain in their own home surrounded by those important to them.

Our Well Being centre provides a rehabilitation program empowering people with palliative conditions to manage their condition, enabling them to live as independently as possible.

Our staff are as important to us as the care we deliver and will receive a robust induction, the opportunity for ambitious continuous development and wellbeing support.

Main duties of the job

1. To be responsible for assessing, planning, implementing and evaluating patient’s care needs and that plans of care are person- centred and holistic using evidence based practice.

2. To supervise, direct and manage the work of all staff and volunteers contributing to undertake patient care so that professional and consistent standards of care are maintained at all times.

3. To ensure that holistic care is provided in accordance with approved policy and procedures and assessed needs.

4. Adopt or implement a team approach to care, recognising the role of the wider team, allocating work to them according to Mary Ann policy.

5. To establish priorities and programmes of care for individual patients and support those who matter to them i.e. family carers.

6. To develop and ensure good communication with all staff liaising with patients, relatives, carers and all other professionals at Mary Ann and other health and social care teams, ensuring quality and continuity of patient care.

7. To be confident with communication skills enabling important discussions with patients, families and professional colleagues in respect of preferences and wishes relating to palliative and end of life care.

8. To represent Mary Ann at fundraising, public and professional events to help promote Mary Ann services as necessary.
