Senior Care Support Worker

Salary: AfC Band 3

37.5 hours per week (08.30 – 16.30, 5 days over a 7 day week) 

This is an exciting time to join Mary Ann as we extend our community services to reach more patients and their families throughout North Warwickshire and the surrounding areas. Our patients and their families are at the heart of everything we do. Mary Ann provides a wide range of multi-disciplinary services, created and designed to support individual needs, which include a virtual day hospice, complementary therapies, community services nursing team, 24hr Rapid Response team and a family support (including bereavement) service. We also provide expert secondary lymphoedema care and treatments.

The clinical services provided by Mary Ann aim to provide assessment, intervention and monitoring of patients with progressive and life limiting illnesses enabling them to remain at home. This includes patients in residential, nursing homes and other long stay settings for as long as possible according to their individual preferences.

About the Role:

The post holder is responsible for supporting the delivery of palliative and end of life care as part of the Mary Ann clinical services team, providing care interventions and on-going assessment under the direct clinical leadership of the staff nurses and team leads.

The post holder will work independently, to assess and report the current patient status, support patients and carers and initiate and deliver specific interventions as required, in accordance with the supporting practice competencies.

The post holder will support patients and carers in their own homes, including residential/nursing homes and other long stay settings for as long as possible, according to their individual preferences.

About you:

The post holder will hold a Care Certificate Qualification and NVQ Level 3 in Health and Social Care, they will be able to demonstrate a commitment to continuing professional development and a willingness to complete further studies.

The post holder will be committed to developing their clinical skills and experience to benefit patients and will have recent palliative and end of life care experience gained whilst working in a care setting.

The post holder will possess strong communication skills to enable them to build effective working relationships with their patients, carers and other healthcare professions at all levels across the healthcare sector, including good IT skills, and be able to demonstrate commitment to ensuring the patients right to be involved in decision making in relation to their health care. The post holder will be able to work independently as well as working as a member of Mary Ann’s professional healthcare team.

The post holder will hold a current driving licence and have the use of a vehicle for duties associated with this role.
